Followed Cloudstack installation manual. Installation went smoothly, but when attempting to login for the first time I got this error message.

Your username/password does not match our records.

Tried default admin/password, but no luck.

Cloudstack 2.2.8
Centos6

ok, fixed the problem ... it may help someone else.

There is "cloud-setup-management" command that has to be run after database setup which I skipped somehow. After I run it and restarted cloud-management service, everything's fine.

I'm having the same problem as well. I have run cloud-setup-database and cloud-setup-management. I have also dropped all the cloud databases in mysql and run them both again. I am still getting the same error no matter what I try.
